What is Sub-acute Care?
Subacute care is comprehensive inpatient care designed for someone who has an acute illness, injury, or exacerbation of a disease process. It is goal oriented treatment rendered immediately after, or instead of, acute hospitalization to treat one or more specific active complex medical conditions or to administer one or more technically complex treatments, in the context of a person's underlying long-term conditions and overall situation.
Subacute care is generally more intensive than traditional nursing facility care and less than acute care. It requires frequent (daily to weekly) recurrent patient assessment and review of the clinical course and treatment plan for a limited (several days to several months) time period, until the condition is stabilized or a predetermined treatment course is completed.
Cardiac Care
Our cardiac care services are available to individuals recovering from a heart attack or bypass surgery, or who may be experiencing abnormal heart rhythms or high cholesterol. In addition, our services such as in-room oxygen and suction are also beneficial to residents with congestive heart failure or heart disease. Our cardiac care services provide nutritional counseling, smoking cessation, psychological interventions, exercise training, physical activity counseling and management of lipid levels, hypertension, weight and diabetes.
The staff at Four Seasons is competent in delivering above excellent sub-acute care to our patients. Our newly renovated state of the art sub-acute unit is equipped to offer many needed services to patients post-hospital stay. Our goal is to return patients back to their current living situation in the community as quickly as possible.
